Making an Appointment

Call our receptionist at (515)-599-8904 or fill out the Contact Us form (under the Locations tab) and we will follow up with you.

What to expect:

  • Our receptionist may ask some questions regarding your concerns.

  • Our receptionist will ask for your insurance card.

  • For payment, we take cash, checks, Mastercard, VISA, Discover, and American Express.

  • If you are using a credit card for payments, the receptionist can take that over the phone or you may fill out the credit card authorization form and bring that to your first appointment.

  • Payment is due at time of service.
